fix: remove duplicate claude.md (case-insensitive filesystem conflict)
Windows treats CLAUDE.md and claude.md as the same file. Remove the lowercase variant to prevent merge conflicts on case-insensitive systems. Co-Authored-By: claude-flow <ruv@ruv.net>
