docs: update user guide with v0.4.1 firmware release and CSI troubleshooting

- Add v0.4.1 to firmware release table as recommended stable release
- Update flash command with correct partition offsets (8MB, OTA)
- Add "CSI not enabled" troubleshooting entry
- Add warning about pre-v0.4.1 firmware CSI bug
